Hello and welcome to our updated Caring Communities of Change Project. By clicking on the map below, you can see updates on the work we are collaborating on across Wales with key stakeholders across the three themes of the project.
Caring Communities of Change is all about parents and carers ideas and creating, sharing and learning, whilst everyone contributes towards to stepping stones to change.
How will we do this?
Caring Communities of Change will put learning disability family carers at the heart of service design and delivery across Health and Social Care in Wales.
As a result of partnerships with carer-led networks and other organisations we will co-produce, pilot and scale up innovative and practical solutions to meet the needs of the learning disability family carers and their families.

The ‘red pins’, represent work that All Wales Forum, parent carers and partners are working on at the moment. You can find out more by clicking on these and they will take you to a detailed update on each of the project strands.
You’re interested but have limited availability. So, how can you get involved? Well, you can get involved by:
- Talking to us – Digital and face to face conversations
- Working with us – Engagement working groups
- Innovating with us – Pilot projects

Creative Carer Communities
The unpaid carer community is a creative one, with parents, carers and carer led networks working together and creating ever more imaginative ways of supporting each other and their families.
