The process of transition is an area of concern for many learning disability family carers and the relatives they support, leading to many learning disability family carers describe this period as one of the most stressful for the whole family.

What is our Goal?
All Moves Matter will generate creative opportunities, enabling families to have greater control of transition planning. In other words, the choices available to them by accessing alternative models and purchase options give families a voice and control over the process.
What does this look like?So, what does this look like? An example of co-production of services to meet the needs of the family would be an example from Wales, where a family work with the local authority to identify positive outcomes for the family, in which the use Direct Payments flexibility. After that, this enabled the family to rent private accommodation near the family home, giving flexibility, freedom, support and control over the transition process, leading to improved well-being for all.

So, how will we do it? In other words, The All Moves Matter strand will focus on three schemes to support transitions options –
Home from Home – LD families facing transition have Shared Lives placement as a meaningful and accessible life option choice for their future Direct Independence – LD Families in Wales facing transition have more meaningful choice and control over Independent Living options, retaining independence for the future Referral Pathway Model – LD Families facing transition are actively enabled to remain in or find a way back to employment and/or life-long learning. |
