Caring Communities of Change will put learning disability family carers at the heart of service design and delivery across Health and Social Care in Wales.
As a result of partnerships with carer-led networks and other organisations we will co-produce, pilot and scale up innovative and practical solutions to meet the needs of the learning disability family carers and their families.

Alternative options to respite and short breaks that are responsive and flexible to the needs of families
Creative Carer Communities – Resilient and inclusive communities
Re-design of community activity to make sustainable changes that meet the needs of the communities.
