Caring Communities of Change

Caring Communities of Change will put learning disability family carers at the heart of service design and delivery across Health and Social Care in Wales.

As a result of partnerships with carer-led networks and other organisations we will co-produce, pilot and scale up innovative and practical solutions to meet the needs of the learning disability family carers and their families.

What is Co-production?

Co-production, what is it? In summary, it is involving citizens in the creation of policies and services. Co-production Network Wales have a great guide to co-production here

What are our goals?

In conclusion, we want to enable Learning Disability families across Wales to –

  • Secure rights for carers and their loved ones

  • Enhance physical, emotional and economic well-being

  • Have an active contribution to society

What will we offer?

So what are we doing and what will we concentrate on?

All Moves Matter – Transitions

Creative opportunities and alternative purchase options to give a voice and control during the process of planning from home to independent living

Break from the Routine – Respite

Alternative options to respite and short breaks that are responsive and flexible to the needs of families

Creative Carer Communities – Resilient and inclusive communities

Re-design of community activity to make sustainable changes that meet the needs of the communities.
